Required Student Athlete Paperwork

Students must complete the school registration process before participating in athletics. As part of registration, parents/guardians must provide authorization for athletic participation, consent to the District’s drug-testing program, and provide current insurance information.

In addition, students must have a current physical on file each school year, and both the student and parent/guardian must complete the required VHS Handbook signatures before the student is eligible to participate.

Yearly Physical: High School Physical Forms   Please print physical paperwork and have your child's doctor fill out. 

 Completed and signed forms must be returned to the VHS office before practice begins.
